Case Manager
Job Summary:
The Case Manager is responsible for providing comprehensive support services to residents of the transitional house. This role involves coordinating care plans, advocating for resident’s needs, and connecting them with resources to support their recovery journey. The Case Manager ensures that residents adhere to their recovery plans while fostering a safe, supportive, and structured environment.
Case Management:
- Conduct initial intake assessments to determine residents’ needs, strengths, and goals.
- Develop individualized service plans in collaboration with residents.
- Monitor progress and adjust transitional plans as needed.
Advocacy and Support:
- Provide one-on-one support to residents, including crisis intervention and conflict resolution.
- Advocate for residents with service providers, employers, and community resources.
- Assist residents with navigating systems such as healthcare, legal services, and social services.
Resource Coordination:
- Connect residents with local resources, including employment assistance, education programs, therapy, and recovery meetings.
- Coordinate transportation for residents to essential appointments, as needed.
- Maintain an up-to-date database of available community resources.
Documentation and Reporting:
- Maintain accurate and confidential records of residents’ progress and services provided.
- Prepare and submit timely reports on case management activities.
- Ensure compliance with organizational policies and regulatory requirements.
Program Facilitation:
- Lead group sessions on topics such as life skills, relapse prevention, and goal setting.
- Support residents in maintaining a structured daily schedule.
- Encourage participation in house activities and compliance with house rules.
Collaboration and Teamwork:
- Work closely with staff, counselors, and external providers to ensure coordinated care.
- Attend regular team meetings and contribute to program development.
- Provide support to the recovery house team, including during emergencies or critical situations.
Education and Experience:
- Bachelor’s degree in social work, psychology, or a related field (required).
- Master’s degree or LSW/LPC (preferred).
- Minimum of 2 years of experience in case management, substance use treatment, or recovery services.
Skills and Competencies:
- Strong understanding of substance use disorders and recovery principles.
- Excellent interpersonal and communication skills.
- Ability to manage multiple cases effectively and prioritize tasks.
- Knowledge of local community resources and support systems.
- Proficiency in maintaining accurate and confidential records.
- Valid driver’s license and reliable transportation.
- Flexibility to work evenings or weekends, as needed.
- Commitment to a trauma-informed, person-centered approach.
The Case Manager works in a residential recovery house setting, which may require responding to emergencies and supporting residents in emotionally challenging situations.

Peer Support Specialist
- Assists individuals with developing their individual service plans and other activities aimed at increasing their active participation in person-centered planning and the delivery of individualized services.
- Shares personal recovery stories and describes personal experiences to inspire and support individuals throughout the recovery process.
- Supports individuals with problem solving related to their reintegration into the community.
- Advises and informs individuals on community resources, including housing, employment, education, transportation, recreation, health, religious/spiritual, and other resources to meet their individual recovery goals.
- Provides individuals with support and guidance on recognizing the early signs of crisis, developing crisis plans, and implementing identified coping strategies.
- Assists individuals with developing and maintaining positive personal and social support networks.
- Assists individuals with developing their self-help skills and cultivates their ability to make informed, independent choices.
- Plans and facilitate practical activities for individuals to increase their self-worth and improve their self-concepts.
- Assists in facilitating psychoeducational groups.
- Attends treatment/recovery team meetings.
- Documents services provided to individuals.
- Acts as an advocate for individuals to facilitate positive changes in their recovery.
- Performs related work as required.
Knowledge of mental health peer support services.
- Knowledge of the recovery process and how it applies to individuals
- Ability to follow written and oral instructions.
- Ability to maintain effective working relationships.
- Ability to communicate effectively orally.
- Ability to communicate effectively in writing.
- Must be a person with a lived experience of mental health or co-occurring mental health & substance use disorder.
- Must satisfy relevant state requirements for Certified Peer Recovery Supporter within 12 months of employment which include: being registered through OhioMHAS as a Certified Ohio Peer Recovery Supporter which is based on application approval including demonstration of successful completion of: 16 hour on-line OhioMHAS E-Based Academy Course, a minimum of 40 hours of peer service delivery training or equivalent 3 years formal experience in peer service delivery, and passing the OhioMHAS Certified Peer Recovery Supporter exam. Certified Peer Recovery Supporters have an identified specialty of mental health and/or substance use disorder based upon their personal recovery experience.

Van Driver
Job Summary:
The Van driver is responsible for safely transporting program participants, supplies, or equipment to various locations as needed, adhering to all traffic laws and company policies while prioritizing the well-being of passengers and representing the non-profit values through professional conduct and positive interactions with the community.
Transportation Services:
- Pick up and drop off individuals at designated locations, including client locations, i.e. prison, treatment centers, program sites, and community events.
- Transport supplies, food, or equipment as required for non-profit activities.
- Follow established routes and schedules for efficient transportation.
Vehicle Maintenance:

- Regularly inspect vehicles for safety concerns, including tire pressure, fluid levels, and lights.
- Report any maintenance issues to the supervisor promptly.
- Maintain cleanliness of the van, both inside and out.
Passenger Care:
-
- Assist passengers with loading and unloading, especially those with mobility challenges.
- Provide a safe and comfortable environment for passengers during transport.
- Communicate clearly with passengers regarding pick-up and drop-off times and locations.
Compliance and Documentation:

- Adhere to all traffic laws and company safety regulations.
- Maintain accurate records of mileage, fuel usage, and passenger details.
- Complete necessary paperwork related to transportation services.
Customer Service:
- Interact professionally with clients, staff, and the public, reflecting the non-profit’s positive image.
- Respond to inquiries and concerns regarding transportation needs in a timely manner to the case manager, program manager or scheduler specified.
- Valid driver’s license with a clean driving record
- Background check
- Proven experience driving a van or similar vehicle
- Excellent customer service skills and ability to interact with diverse individuals
- Strong understanding of local traffic laws and regulations
- Ability to lift and carry moderate loads when necessary
- Commitment to safety and a focus on client well-being
- Availability to work on a non-transitional schedule, including evenings and weekends depending on program needs
